Job Summary
Deliver skilled, patient-centered care to home health patients in the North Side, PA area, performing assessments, medication management, and care planning while educating families on disease management. Think critically to solve real-time problems and ensure thorough, compliant EMR documentation. Work independently within an assigned territory, traveling to patient locations using a provided fleet vehicle for approximately 20% of your day. This full-time weekend role offers a $15,000 total bonus potential, including a $10,000 first-year success bonus and a $5,000 sign-on bonus. You must hold an active RN license, one year of RN experience, and a current driver's license.
