Job Summary
Deliver skilled, patient-centered care in a home health setting by performing patient assessments, managing medications, and creating care plans for patients in the Erie, PA area. Educate families on disease management and treatment plans while ensuring thorough, compliant EMR documentation. Travel to patient homes within your assigned territory, working independently but collaborating with an interdisciplinary team. This full-time weekend position includes a 40-hour work week and offers up to $20,000 in total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on bonus. You must hold an active RN license with one year of experience and a current driver's license. Required Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Legally authorized to work in the United States
