Job Summary
Deliver skilled, patient-centered care in a home health setting by performing patient assessments, managing medications, and creating care plans while educating families on disease management. Collaborate with an interdisciplinary team to think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently within your assigned territory, traveling to patient homes to provide direct support, utilizing your current driver's license for approximately 20% of your day spent driving. Required Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience
- Current driver's license
