Home Health RN Registered Nurse Part Time Weekends

Deliver one-on-one skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, and medication management while educating patients and caregivers on symptom management. Monitor patient progress, communicate with physicians and the interdisciplinary team to adjust plans, and coordinate care across providers to ensure continuity and prevent hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Commit to Saturday and Sunday visits with initial training during the week, requiring a current driver's license, reliable transportation, and auto insurance.
