Home Health RN Registered Nurse Part Time Weekends

Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, catheter care, and medication management, while educating patients and caregivers on symptom and daily care management. Monitor patient progress, communicate with physicians and the interdisciplinary team to adjust plans, and coordinate care across providers to ensure continuity and prevent avoidable hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Commit to Saturday and Sunday visits with initial training during the week in Brevard County.
