Home Health RN Registered Nurse Part Time

Deliver one-on-one skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, catheter care, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to identify changes early and prevent hospitalizations. Coordinate care across providers to ensure continuity and complete accurate documentation supporting quality and compliance. Requires an active RN license with at least one year of clinical experience, a current driver's license, and CPR certification. Part-time schedule available.
