Home Health RN - Per Diem

Conduct initial patient and family assessments to determine home care needs, including complete physical assessments and histories of current and previous illnesses. Re-evaluate nursing needs every four weeks, initiate care plans with goals, and incorporate therapeutic, preventive, and rehabilitative actions while involving patients and families in planning. Administer medications and treatments as prescribed, counsel patients on meeting nursing needs, provide health care instructions, and identify discharge planning needs prior to discharge. Act as Case Manager when assigned to coordinate patient care for the caseload.
