Job Summary
Complete patient assessments, develop individualized care plans, and perform OASIS assessments while maintaining accurate documentation. Provide skilled nursing care, treatments, and medication administration, and educate patients and families on care and disease management. Coordinate care with physicians and interdisciplinary teams to support discharge planning and ongoing patient needs. This role offers flexible part-time or PRN scheduling with weekly and same-day pay options.
