Job Summary
Complete patient assessments, develop individualized care plans, and perform OASIS assessments while maintaining accurate documentation. Provide skilled nursing care, treatments, and medication administration, then educate patients and families on disease management. Coordinate care with physicians and interdisciplinary teams to support discharge planning and ongoing patient needs. This part-time or PRN role offers flexible scheduling with weekly and same-day pay options.
