Job Summary
The Home Health RN delivers and supervises individualized patient care within a home-based setting, following the nursing process and the Nurse Practice Act. Responsible for assessment, treatment, and coordination of the patient’s plan of care in collaboration with the health-care team, involving families as appropriate. Directs and oversees care provided by colleagues, promotes safe practices, documents electronically, and participates in discharge planning. Works within a collective-bargaining environment, with day shift, full-time schedule, and responsibilities that emphasize patient advocacy, education, and ongoing professional development; must maintain licensure, certifications (including BLS), and adherence to safety standards. The role supports continuous quality improvement and interprofessional collaboration to optimize patient outcomes in home-health services.
Required Qualifications
- Graduate of an accredited/approved school of Nursing
- WA: Required - Associate's Degree Nursing OR: Required - Education to meet certification, license or registration requirement
- WA: Required upon hire: Washington Registered Nurse License
- Required within 30 days of hire: National Provider BLS - American Heart Association
- Required within 30 days of hire: National Provider Identifier (NPI) number
