Home Health RN - Morehead City, NC

Provide admission, case management, and follow-up skilled nursing visits for home health patients as directed by the Physician and Clinical Manager. Coordinate care with physical therapists, occupational therapists, licensed practical nurses, and external providers. Report patient conditions and progress to the physician and Clinical Manager. Prepare medical documentation including case conferences, medication order changes, re-certifications, and care plan updates. Submit visit reports and shift summaries to support optimum patient care. Requires an alternative Baylor/Weekend full-time schedule.
