Job Summary
Home Health Nurse (RN) provides admission, case management and follow-up skilled nursing visits for home health patients, coordinating care with Physical Therapists, Occupational Therapists, LPNs, Home Health Aides, and external providers. Maintains documentation including care plans, medication changes, re-certifications, and visit/shift reports; reports patient status to physician and Clinical Manager; collaborates with health care teams to support patient recovery and independence. Requires NC RN license, valid driver’s license with auto insurance, and CPR certification. Schedule includes an alternative Baylor/Weekend full-time arrangement (12-hour shifts). Pay range $77,200–$106,200 annually; sign-on bonus of $10,000 for new associates.
