Job Summary
Provide skilled nursing care in the home following discharge from a hospital, rehab, or nursing facility. Conduct assessments, document progress, and communicate changes to the physician. Oversee LPNs, CNAs, and aides supporting your patients. Educate patients and families on care plans, medications, and home safety. Coordinate care goals and manage discharge from services. Strong clinical judgment, communication skills, and a genuine passion for patient-centered care. Active RN license in Oklahoma and CPR certification required; at least one year of nursing experience; home health experience preferred. Reliable transportation.
