Job Summary
Conduct comprehensive patient assessments upon admission and continually reassess status to apply appropriate interventions and advocacy. Collaborate with patients, families, and the healthcare team to develop, document, and monitor individualized plans of care that incorporate family-centered care principles. Direct and delegate care delivered by colleagues while performing procedures and treatments according to departmental protocols and the Nurse Practice Act. Legibly and electronically document all patient care activities accurately and timely, utilizing tools like the Providence Early Assessment Team when indicated. Monitor patient conditions, note changes in status, and communicate updates via S.B.A.R. format to physicians and colleagues. Support orientation of new staff and students, and actively participate in quality improvement programs, departmental activities, and scheduled staff meetings.
Required Qualifications
- Graduate of an accredited/approved school of Nursing
- Associate's Degree Nursing
- Bachelor's Degree Nursing
- 1 year Registered Nurse experience in an acute care hospital
- 1 year Professional nursing experience
- 1 year Experience in home health, acute care or a skilled nursing facility
- 2 years Experience in acute care or community mental health setting
- 1 year Recent home health, acute care or skilled nursing facility
- Alaska Registered Nurse License
- California Registered Nurse License
- Oregon Registered Nurse License
- Washington Registered Nurse License
- National Provider BLS - American Heart Association
- Driving license
- Auto insurance
- National Provider Identifier (NPI) number
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Regular standing, walking, and manual dexterity
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Ability to prioritize multiple tasks

- Attends and participates in a minimum of 50% of scheduled staff meetings or has read the minutes and signed off on them

Desired Qualifications
- Bachelor's Degree Nursing
- 1 year One year Registered Nurse experience in an acute care hospital
- Home Health field experience
- IV and Central Line experience
- Experience with wound care including negative pressure devices
- Recent Home Health experience
- Hospice or Palliative Care experience
