Job Summary
RN Case Manager in Home Health providing skilled nursing care in patients' homes. Responsible for assessing, planning, implementing, and evaluating individualized plans of care in collaboration with physicians and the interdisciplinary team; completing admissions and ongoing nursing assessments; educating and supporting patients and families through changing needs; and documenting care accurately. Requires an active Missouri RN license and ability to manage a flexible schedule; home health experience preferred. Salary range $72,000–$90,000 based on experience; full benefits, PTO, 401(k) with match, and flexible scheduling available.
Required Qualifications
- Active Missouri RN license in good standing
- Home health RN experience preferred
- Strong clinical judgment and documentation skills
- Ability to manage a flexible schedule including admissions
- Comfort with technology and electronic medical records
