Home Health RN (Full Time) in Cullman, AL. Evaluate and admit patients to home care; develop plans of care and provide care accordingly. Act as case manager to adjust plans for symptom changes and care needs; review and plan patient care and discharge during case conferences; serve as liaison for patient/caregiver with physicians and community healthcare partners. Strive to lower cost of care through mileage, supplies, and visit scheduling; complete all documentation related to patient contact, assessments, SOC, and recertification; ensure resumption of care and discharge per policy; maintain understanding of current home care regulatory guidelines; provide on-call coverage for home care and hospice as assigned; promote teamwork, patient safety, and quality. Requires ADN or higher, Alabama RN license, and at least 2 years hospital experience; strong critical thinking, organizational skills, independent decision-making, flexibility in scheduling, and ability to manage and prioritize patient needs.
