Job Summary
Deliver skilled, patient-centered care in a home health setting by performing patient assessments, managing medications, and creating care plans. Educate patients and families on disease management and treatment plans while thinking critically to solve problems and ensure compliant EMR documentation. Work independently within an assigned territory, traveling to patient homes to provide care, collaborate with an interdisciplinary team, and spend approximately 20% of your day driving. Requires an active RN license, one year of RN experience, and a current driver's license. This role is part of AHN Healthcare@Home's mission to help patients maintain independence and continue healing at home through home health, hospice, and palliative services.
Required Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Legally authorized to work in the United States
