Job Summary
Deliver skilled, patient-centered care in a home health setting by performing patient assessments, managing medications, and creating care plans. Collaborate with an interdisciplinary team to educate and empower patients and families on disease management while ensuring thorough, compliant EMR documentation. Work independently within an assigned territory, traveling to patient homes in the Green Tree and Crafton areas, with approximately 20% of your day spent driving. This role offers a $10,000 first-year success bonus and requires one year of RN experience, a current driver's license, and an active RN license in the state of employment.
Required Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Legally authorized to work in the United States
