Job Summary
Deliver skilled, patient-centered care in a home health setting by performing patient assessments, managing medications, and creating care plans in collaboration with an interdisciplinary team. Educate and empower patients and families on disease management and treatment plans while thinking critically to solve problems in real time. Ensure thorough, compliant EMR documentation and work independently within your assigned territory, traveling to patient homes to provide this support. Requires an active RN license, one year of RN experience, and a current driver's license to spend approximately 20% of your day driving. This role offers a $10,000 first-year success bonus and includes medical, dental, vision, and pet insurance, a fleet vehicle program, and a Purdue University partnership for up to 90% off degrees and certifications.
Required Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience
- Clinical expertise to deliver quality patient care
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successfully complete a drug screen
- Be legally authorized to work in the United States
