Job Summary
Home Health RN position in Pottstown, PA supporting patients in the local area. The RN case managers lead and collaborate to provide management and delivery of patient care plans within a team-based home health and hospice environment. Eligible for a $10,000 first-year success bonus. Responsibilities include delivering quality patient care, coordinating with clinicians, and using tablets/mobile devices and EMR software for charting. Requires RN license in the state, 1 year of RN experience, a valid driver’s license, and the ability to drive ~20% of the day to patient locations. Required Qualifications
- RN license in the state you work
- Graduate from an approved school of practical nursing
- One year of RN experience and the clinical competence to deliver quality patient care
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- Demonstrated experience with tablets, mobile phones and EMR software
