Job Summary
Provide in-home nursing care as a Home Health RN Field Nurse, building from community resources to plan and direct home-based services. Conduct initial and ongoing patient assessments, perform comprehensive physical assessments, supervise the LPN and Home Health Aide in implementing the care plan, develop and modify patient education and care plans, administer medications as prescribed, coordinate discharge planning, and act as case manager when assigned by the Clinical Supervisor. Develop goals based on nursing diagnosis and establish therapeutic, preventive, and rehabilitative actions to support patients’ independence at home; prepare clinical notes and communicate with physicians regarding patient needs.
Required Qualifications
- Graduate of an accredited school of nursing
- Active RN licensure
- One to two years of recent acute care experience
- Bachelor’s degree with one year of home health care experience preferred
- Must be a licensed driver with an automobile insured per requirements
