Job Summary
Home Health RN responsible for performing initial patient and family assessments in the home, monitoring and re-evaluating nursing needs, and initiating and revising the plan of care as patient status changes. Role focuses on intermittent home health, with exposure to palliative and hospice care; requires strong clinical nursing and customer service skills, ability to collaborate with families, and adherence to care protocols. Required Qualifications
- Must possess current Illinois RN licensure
- Home Health experience preferred
- Current CPR
- Valid IL driver’s license and auto insurance policy
- HCHB experience a plus
- Must possess excellent clinical nursing and customer service skills
