Job Summary
Deliver skilled, patient-centered care in a home health setting by performing patient assessments, managing medications, and creating care plans. Educate patients and families on disease management and treatment plans while thinking critically to solve real-time problems and ensure compliant EMR documentation. Work independently within an assigned territory, traveling to patient homes to provide skilled nursing care in collaboration with an interdisciplinary team. This full-time evening position in Natrona Heights, PA, offers a first-year success bonus and includes a fleet vehicle program.
Required Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Legally authorized to work in the United States
