Home Health RN

Provide home-based nursing care to patients, including comprehensive assessments, development and implementation of individualized plans of care, and ongoing coordination with the health care team. Teach and involve patients and families in the plan of care, document care activities accurately, and promote patient safety and infection control. Collaborate with team members, participate in discharge planning, and support preceptor and charge nurse responsibilities when applicable. Must hold an Associate's Degree in Nursing and California RN license with BLS certification; 1 year of professional nursing experience preferred; IV and wound-care experience may be preferred.
