Home Health RN Clinical Supervisor -Hybrid

Supervise all patient care services and staff including RNs, LPNs, OTs, PTs, and HHAs while coordinating referrals and ensuring care remains compliant with individualized plans of care. Provide direct patient care when staffing requires, support clinicians with OASIS documentation and case management, and lead clinical meetings and in-services. Review clinical documentation to ensure chart accuracy and regulatory compliance, while monitoring clinical outcomes and conducting ongoing assessments of patient needs. Maintain accurate records and oversee all personnel to uphold agency standards.
