Home Health RN Clinical Manager

Lead clinical operations for CenterWell Home Health in Lillington, NC, overseeing patient care delivery, staff management, documentation quality, and regulatory compliance onsite. Review referrals and admissions, assign clinicians, and ensure Plans of Care meet patient needs and agency standards. Educate and support clinicians in care planning and clinical decision-making; remain available for clinical support. Ensure documentation, audits, and billing meet Medicare, payer, and company standards; participate in hiring, training, performance evaluation, coaching, and corrective action. Coordinate communication among physicians, team members, and caregivers to support care coordination, discharge planning, and outcomes achievement. Participate in quality improvement, data tracking, budgeting, marketing, and community relations. May provide direct patient care in exceptional circumstances and act as Branch Director in their absence. Full-time schedule (40 hours) with on-site duties at the Lillington, NC branch; requires RN license, CPR certification, valid driver’s license, and transportation; 2+ years RN experience in home-based care; OASIS and HCHB experience preferred.
