Job Summary
Home Health RN Case Manager coordinates and directs home health services, developing and initiating the plan of care in partnership with the patient, caregiver, and interdisciplinary team. Responsibilities include patient assessment, initiation of nursing procedures, patient/family counseling and education, communication with physicians and other health-care practitioners, discharge planning, and supervision of home health aides, LPNs/LVNs, and CNAs. Requires RN licensure in the practicing state, CPR certification, and a driver’s license; experience in public health or home care nursing preferred. Per Visit Pay model in Cortez, CO area. Required Qualifications
- Must have graduated from an approved school of nursing and be licensed in the state where practicing.
- Minimum of two years of experience, at least one of which is in public health or home care nursing is preferred.
- Maintains a current CPR certification.
- Must be a licensed driver with an automobile that is insured and in good working order.
