Job Summary
The Home Health RN Case Manager plans, coordinates, and delivers quality home health care in compliance with standards and regulations. Essential duties include ongoing patient assessments in homes and facilities, develops and implements comprehensive care plans addressing current needs, identifies additional services when needed, communicates with other healthcare providers, educates patients and caregivers, oversees plan implementation, and evaluates effectiveness. Also maintains accurate records, supports quality improvement projects, and stays informed about regulatory and payer requirements. Requires an Ohio RN license, at least one year of acute/medical-surgical nursing experience, proficiency with Windows/Microsoft Office and EMR systems, a valid driver’s license, and the ability to work in a home-health setting with variable schedules including on-call. May involve travel across Ohio communities served by Ohio Living Home Health & Hospice and Ohio Living Palliative Medicine.
Required Qualifications
- Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required
- One year experience in acute care or medical-surgical nursing required
- Experience in a Medicare-certified home health agency preferred
- Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required
- Proficiency with electronic medical records (EMR) preferred
- Must be able to read, write, speak, and understand the English language
- Must possess a valid driver's license and acceptable driving record
- Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards
- Working Conditions and Special Requirements: Sitting up to 4 hours/day; Standing up to 8 hours/day; driving up to 6 hours/day; on-call 24/7; exposure to infectious waste and bloodborne pathogens; ability to lift up to 50 pounds
