Job Summary
RN Case Manager delivers patient care through nursing process in home health, supervises Home Health Aides and LPNs, develops plans of care with client/family, manages medications and orders per physician instructions, coordinates with physicians and other disciplines, conducts client assessments, and engages in discharge planning and quality assurance activities. Requires RN licensure and typically at least 1 year of home health experience with strong communication and problem-solving skills; offers benefits and telehealth options.
Required Qualifications
- Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided.
