Home Health RN Case Manager - La Grange and Surrounding Area

Provide nursing care for clients of all ages in the patient’s residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; assess and implement the individual plan of care, educate patients and families on disease processes and end-of-life care, train staff as needed, and coordinate services with the IDT; responsibilities include comprehensive assessments, planning and revising the care plan, documenting progress, and ensuring patient/family needs are met through coordination and communication.
