Job Summary
Complete nursing initial and ongoing assessments of in-home clients within specified deadlines, performing procedures requiring specialized knowledge and judgment. Participate in the development and evaluation of care plans in partnership with patients, representatives, and caregivers, while overseeing patient case loads through ongoing interdisciplinary care coordination. Perform patient home visits to evaluate care quality and review clinical notes, conducting ongoing interdisciplinary assessments to ensure comprehensive support. Maintain knowledge of state and federal home health regulations and demonstrate strong communication skills and compassion throughout the process.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Colorado
- Minimum of one (1) year of nursing experience
- CPR Certification (demonstration course required)
- Pass a background check and drug test
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty
- Strong communication skills
- Compassion
