Job Summary
Complete nursing initial and ongoing assessments of in-home clients within specified deadlines. Perform care plan oversight by participating in ongoing interdisciplinary care coordination and evaluation of the plan of care in partnership with patients and caregivers. Conduct patient home visits to evaluate care quality and review clinical notes. This role requires a Colorado-licensed RN with at least one year of home healthcare experience and CPR certification. Full-time employees in Pueblo, CO earn $50–$130 per visit plus travel time and mileage reimbursement. Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Colorado
- Minimum of one (1) year of nursing experience
- CPR Certification (demonstration course required)

- Clinical proficiency in nursing specialty
