Job Summary
Conduct initial and ongoing patient assessments to determine home care needs and develop individualized care plans. Administer medications and treatments as prescribed while providing skilled nursing care and guidance to patients and families. Coordinate care across services by collaborating with physicians and community providers to optimize outcomes. Maintain accurate clinical documentation and communicate patient status changes to the primary physician. Supervise home health aides bi-weekly to ensure quality care delivery and provide tailored health instructions. Requires current RN licensure, acute care experience, and a licensed driver's license. Flexible scheduling available.
Required Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting
- Current RN licensure
- CPR certification
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Licensed driver
- Automobile that is in good working order
- Automobile insured in accordance with the organization requirements
