Home Health RN Case Manager (full time)

Job Summary
Provides skilled nursing care to patients in their place of residence under physician orders and plan of care, including initial evaluation, developing and revising the care plan, coordinating with the IDT, and delivering preventive/rehabilitative nursing procedures. Maintains timely documentation and progress notes, conducts patient and family counseling, supervises LPN/LVN and Home Health Aides as assigned, participates in peer review and quality management, completes OASIS assessments and related documentation, and performs on-call duties as needed. Requires active nursing license, graduation from an approved nursing program, and ability to exercise initiative; 1 year nursing experience preferred. Certifications and adherence to agency policies are required.
Required Qualifications
- Graduate of an approved school of professional nursing
- Licensed in the state(s) in which practicing
- One (1) year nursing experience, preferred
- Acceptance of philosophy and goals of this Agency
- Ability to exercise initiative and independent judgment
