Job Summary
A Home Health RN Case Manager administers skilled nursing in patients' residences under physician orders and the agency's plan of care, with supervision by the Clinical Manager. Responsibilities include performing initial evaluations, developing and revising the plan of care in coordination with the Interdisciplinary Team, reevaluating patient nursing needs, initiating care plans and revisions, delivering specialized nursing services, completing timely documentation (clinical notes, progress summaries, OASIS, discharge summaries), coordinating services with physicians and caregivers, counseling patients and families, supervising LPN/LVN and Home Health Aides, participating in peer review and quality management, and maintaining agency policies and CPR certification; on-call nights, weekends, and holidays are required.
Required Qualifications
- Graduate of an approved school of professional nursing
- Current RN license in state practicing
- One year nursing experience preferred
- Ability to exercise initiative and independent judgment
- Acceptance of philosophy and goals of this Agency
- Provides services per plan of care
- Participates in IDT and care coordination
- Supervises LPN/LVN and HHA/CNA as required
- Maintains CPR certification
- Completes OASIS assessments and documentation
- On-call duties as assigned
