Job Summary
Assess patients' physical and psychosocial needs, establish treatment plans, and deliver comprehensive home health services including assessments, interventions, and supportive care. Manage assigned caseloads by triaging visits and messages, reconciling medications, and completing physician orders and OASIS documentation. Collaborate with patients, families, physicians, and community resources to coordinate care and discharge planning while advocating for patient safety. Full-time, 40-hour day shift (8am-4:30pm) with weekend/holiday commitment. Located in Merrimac, Amesbury, Salisbury, Newburyport, and surrounding Massachusetts areas.
Required Qualifications
- Massachusetts RN Licensure
- Valid state issued Driver's License
- One (1) year of acute medical/surgical nursing experience
- Cardiopulmonary Resuscitation (CPR) Certification
- Advanced knowledge of job area typically obtained through advanced education and work experience
- Ability to direct the work of other lower level professionals or manage processes and programs
- Assessing, planning, implementing, and evaluating the care needs of a designated group of patients
- Providing professional nursing care to patients
- Interacting courteously and effectively with patients, their families, coworkers, community resources, and physicians
- Maintaining safety in the home and the community at large
- Overseeing the design, implementation or delivery of processes, programs and policies
- Managing projects / processes
- Working independently with limited supervision
- Coaching and reviewing the work of lower level professionals
- Resolving difficult and sometimes complex problems
- Assessing patients' physical, psychosocial needs in a sensitive, caring manner
- Assessing patient/family learning needs, style and limitations
- Establishing realistic goals and developing plans of treatment
- Adapting to new and unusual situations without affecting work performance negatively
- Utilizing Security when and if needed for any potential unsafe situations
- Collaborating with patient /family and other health care providers and/or community resources with planning of care and discharge
- Completing physicians' orders, levels of care, and OASIS on all patients assigned
- Reconciling medications with patient and physician consistently
- Demonstrating ability to cope with patient/family emotional stress and provide appropriate supportive care
- Effectively managing assigned caseload, within the team model of care delivery
- Establishing a daily work plan based upon patient/family priorities of service and total area needs
- Promptly triaging patient visits, messages, and phone calls according to priority and urgency
- Demonstrating excellent physical assessment and care planning skills
- Demonstrating current knowledge of pharmacology and medication administration and reconciliation
- Acquiring and maintaining an expert working knowledge of all third part payers and regulatory bodies and agency policies on issues related to documentation and care provided
- Communicating and collaborating with all disciplines in the home care setting on a regular basis or immediately if there are any critical needs or crisis interventions needed
