Home Health RN Case Manager

Deliver high-quality care to patients with complex needs, including IV infusions, wound vacs, and pain management. Conduct comprehensive initial and ongoing assessments to develop individualized care plans. Coordinate care and communicate effectively with physicians, healthcare professionals, and community resources. Provide education, guidance, and emotional support to patients and their families. Administer medications and treatments as prescribed. Promote patient independence and support positive health outcomes through compassionate, evidence-based care. Full-time, permanent day shift with approximately 50-60 miles of territory travel daily.
