Job Summary
RN Home Health Case Manager delivering in-home skilled nursing care; manage a dynamic caseload of patients in the local community, perform assessments and develop individualized care plans, provide hands-on nursing and patient education, monitor progress, and collaborate with physicians and interdisciplinary teams. Requires active RN license, 1+ year nursing experience, ability to travel locally for home visits, a valid driver’s license and reliable vehicle, and the ability to assist patients and lift up to 50–100 lbs. Required Qualifications
- Active, unrestricted RN license
- At least 1 year of clinical nursing experience
- Ability to travel locally (field role)
- Valid driver’s license, reliable vehicle, and current auto insurance
- Ability to safely assist patients (lifting, positioning, etc.)
- Lift 50–100 lbs intermittently (physical stamina)
