Job Summary
RN Case Manager provides, coordinates, and monitors agency client health care services; ensures regulatory requirements are met; supervises Home Health Aides and Homemakers; coordinates admissions, transfers, and ongoing client assessments; communicates health changes to physicians and families; manages documentation in EMR, oversees on-call nursing coverage, and promotes agency services to discharge planners and physicians; requires MN RN license and accredited nursing education, with strong clinical and coordination skills, telemonitoring capabilities, and a commitment to quality of life for geriatric clients.
Required Qualifications
- Graduate of an accredited school of nursing
- Current Minnesota RN registration
- Strong nursing skills
- Home Health experience preferred
- Basic computer skills
- Excellent communication and time management skills
- Ability to supervise and coordinate
- Confidentiality
- Valid driver's license and automobile insurance
