Job Summary
RN Case Manager responsibilities include providing direct client care in home settings while overseeing home health services, collaborating with physicians and the home health team to update care plans at required intervals, advocating for clients, ensuring documentation accuracy for payer/billing needs, educating clients and families, and exercising clinical leadership over licensed and unlicensed staff. Travel to client homes is required, with 1:1 client care typically lasting 1–3 hours, and scheduling is managed during office hours with on-call rotation. The role supports delivering care to clients across Buffalo and surrounding communities (St. Louis Park, Plymouth, Crystal, Hopkins, and nearby areas) with a focus on quality outcomes, safety, and adherence to the Minnesota Nurse Practice Act, while managing resources and maintaining effective professional relationships. The position offers a base salary range of $70,000–$95,000 plus productivity and benefits, with mileage reimbursement and PTO.
Required Qualifications
- Currently licensed as a Registered Nurse (RN) in Minnesota
- Driver’s license and access to reliable automobile for travel to client homes
- Proof of negative TB testing (Mantoux skin test, TB blood test, or chest x-ray)
- U.S. Citizenship or evidence of valid Alien Work Permit
- Pass initial and ongoing background checks including Minnesota Dept. of Health and OIG List of Excluded Individuals and Entities
- Ability to drive to multiple client homes and travel between service sites
- 2+ years of experience in a health care facility or home care setting
- Good communication and teaching skills; ability to work independently and with diverse teams
- CPR certification (highly recommended)
- Experience with Electronic Medical Records (EMR) and Microsoft Outlook preferred
