Job Summary
Coordinate and manage care for home health patients by providing skilled nursing services, including wound care and medication administration, and developing/updating individualized care plans. Supervise home health aides and coordinate with the interdisciplinary team, ensuring regulatory compliance and thorough documentation. Communicate with patients, families, and physicians to discuss progress and treatment plans; participate in quality improvement and continuing education to maintain current nursing practices.
Required Qualifications
- Current Registered Nurse (RN) license in the state of practice
- Minimum of 2 years of Nursing experience preferred
- Current CPR certification
- Knowledge of OASIS documentation system preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation for home visits
- Physical requirements including lifting up to 50 pounds and ability to navigate variable home environments
