Job Summary
Home Health Registered Nurse (RN) Case Manager in Hampstead, NC, delivering in-home skilled nursing care. Responsibilities include assessing and monitoring patients in their home environment, developing and implementing individualized care plans, educating patients and families on health management, collaborating with a multidisciplinary team to achieve optimal outcomes, and documenting in an EMR while traveling within the service area. Full-time, Monday–Friday, 8am–5pm, with travel to patient homes; requires RN licensure and strong assessment/documentation skills. Required Qualifications
