Job Summary
Provide expert home health nursing care in patients' homes on weekends. Conduct comprehensive patient assessments, develop and implement individualized plans of care, and deliver outcome-driven interventions under physician-ordered Plan of Care. Collaborate with an interdisciplinary team to ensure safe, high-quality, patient- and family-centered care; document electronically and participate in ongoing staff education. Requires RN license and Associate or Bachelor's degree from an accredited School of Nursing; BLS certification is required. Weekend, part-time role with day shift on Saturdays and Sundays; supports orientation of new staff and potential preceptor/charge nurse responsibilities. Familiarity with the nursing process, advocacy across diverse age groups, and commitment to evidence-based practice in home-based care.
Required Qualifications
- Current, unencumbered RN license in the state of employment
- Associate or Bachelor's degree from an accredited School of Nursing
- BLS certification (American Heart Association)
