Job Summary
Develop and initiate individualized plans of care in partnership with patients, caregivers, and physicians. Perform preventative and rehabilitative nursing procedures, provide patient and family counseling, and prepare clinical notes. Coordinate care with community health partners, update the primary physician every sixty days, and identify discharge planning needs. Supervise home health aides, licensed practical nurses, and certified nursing assistants, ensuring equipment availability and participating in quality assessment programs. Maintain current CPR certification, hold a valid driver's license, and work weekends in Davis and Weber Counties at rates between $45.00 and $110.00 per visit.
Required Qualifications
- Graduated from an approved school of nursing
- Licensed in the state where practicing
- Minimum of two years of experience
- Current CPR certification
- Valid driver's license
- Automobile insurance coverage
- Access to an automobile
- Self-directed and able to work with minimal supervision
- Excellent observation skills
- Problem solving skills
- Verbal and written communication skills
- Nursing skills per competency checklist
- Ability to organize and prioritize workload independently
- Ability to lift, position, or transfer patients
- Able to lift supplies and equipment
- Visual acuity and hearing to perform required nursing skills
