Home Health - Registered Nurse (RN) - Torrance

Conduct home health visits to administer services, perform comprehensive assessments, and update patient treatment plans. Administer IVs, change dressings, clean wounds, and document all activities via Synergy Software. Obtain physician orders, coordinate care with the Clinical Team Manager and other team members, and manage re-certifications, transfers, and discharges. Supervise Home Health Aides and assess needs for physical, occupational, and speech therapy support. Maintain regulatory compliance and communicate significant patient condition changes to the assigned team.
