Home Health Registered Nurse (RN) - Skilled Nursing

Provide skilled nursing care based on each patient's individualized Plan of Care, including wound care, VAC management, IV antibiotic administration, and infusion treatments. Educate and counsel patients and families throughout the care process while maintaining accurate clinical documentation per local, state, and federal regulations. Communicate changes in patient condition to the Director of Nursing and appropriate staff. Perform skilled visit treatments as required with a visit quota of 25 weekly. Rotation on-call is required for this full-time role in the RGV area. Training provided if needed.
