Home Health Registered Nurse RN SE

Registered Nurse (RN) for home health in the Southeast Greater Houston area. Provides services per the patient’s plan of care, conducts initial evaluations and regular re-evaluations, delivers skilled nursing with a focus on wound care, IV therapy, and IV management, and coordinates with multidisciplinary team members. Documents care using Kinnser software, initiates preventative and rehabilitative nursing skills, prepares clinical notes and care summaries, communicates patient changes to the supervising team, counsels patients and families, participates in in-service programs, adheres to agency policies, completes orders and discharge summaries, determines nursing needs per patient, and may supervise other nursing personnel.
