Home Health Registered Nurse RN PRN Part time

Develop a plan of care through physician orders, client input, and nursing assessment to include medical interventions and measurable goals. Educate clients and family members based on specific needs while properly orienting and training primary caregivers. Provide skilled nursing care in patients' homes using skills such as phone triage, wound care, infusions, and catheter management. Coordinate the continuum of hands-on client care through documentation and timely communication with physicians and other caregivers. Adhere to confidentiality standards and professional boundaries while documenting care in accordance with the developed plan. Schedule includes Mon-Fri with one weekend on-call rotation.
