Home Health Registered Nurse RN PRN Part Time

Deliver individualized, one-on-one nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide skilled interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to identify condition changes early. Coordinate care across providers to ensure continuity and adjust plans to prevent avoidable hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Serve as the primary clinical partner for patients and families in Oshkosh, Neenah, and Menasha. Requires an active RN license with one year of experience, CPR certification, and a valid driver's license.
