Home Health Registered Nurse RN PRN Part Time

Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Deliver individualized, one-on-one nursing care including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate and empower patients and caregivers through clear instruction, while monitoring progress to identify changes and communicate effectively with physicians to prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation supporting quality and compliance. Serve as the primary clinical partner for patients and families in Appleton, Neenah, Menasha, and New London on a PRN part-time basis Monday through Friday.
